London: Printed for J.T. and sold by Tho. Astley at the Dolphin and Crown in St. Paul's Church-yard, 1726.

"A physician from Yorkshire educated at Peterhouse, Cambridge. He was physician to King George I, and physician-general to the army. He wrote The Dispensary, a poem containing a lively satire on apothecaries and physicians" (A Dictionary of the History of Medicine).
